Request Forms

OSIS Forms are flexible, the forms are designed to be dynamic. OSIS Administrators can build their forms from a blank page by adding components and questions.

Create or Edit an Request Form

Open the OSIS Admin page (/osis/admin/) and scroll down to the Request Section. Click Request Forms and then Click Add to start the creation process, or select one of the existing forms to open up the edit form.

The General tab contains basic details of the request form.

The Asset Type field indicates which asset type will be allowed on the form.

A Request Type must be specified in order to save the form.

Adding Location Restrictions to the form

By default there is no Location restrictions placed on request forms.

We can also apply restrictions by Location Classifications, for example, only allow rooms classified as Classrooms and Computer rooms.

Adding location restrictions will force the end user to only be able to select from locations entered. When assigning what to restrict to, use the lowest common level. If a form is to be restricted to only selected rooms in a building, only enter the rooms. If you want to restrict to specific floors, only enter the floors. If you want to mix restrictions, for example, all of Building A, Floor 2 of Building B and 3 rooms on the 4th floor of building C, then your restrictions would list the following:

  • Under buildings would be Building A
  • Under Floors would be Building B floor 2
  • Under Rooms would be Room 401 from Building C.
  • Do not add the Building B to the Building level restriction or add and Building C values under building or floor.
